The staff is overworked and thin in most of these facilities.
While those in direct contact with individuals who need care are dedicated to those under their care, it is the "upper caste" who do everything they can to cut costs. Those who need care suffer, usually in silence.
Most states could care less and do little to ensure proper care. I worked for a few after I got out of the Army (I was a Medical Plt Sgt) and had no problem calling the "powers that be" at the state level. There are some companies that own virtually all of the Care Facilities in a state. It came down to "if you save a dollar, do it."
