Yesterday

It keeps track of appointments, both future and past, as well as test results, doctor's notes and summaries, prescriptions, and more. I can also send messages to my doctors in it.

I had to download and install it on my phone. It's used by a lot of different medical centers.

Maybe you could ask if there's anything like that available to you?
