The answer appears to be 'no' you don't explicitly need a photo-id to get a REAL ID, though for the vast majority of people, it's really just an add-on status to your existing DL or State ID card, where you get a gold star in the upper right that means 'you're confirmed to be a citizen or permanent legal resident'. However permanent legal resident's don't have Federal voting rights so this is likely why REAL ID is not considered 'good enough' unless it has some special 'citizen-specific' markings, which can vary by state apparently

The Guardian says Real IDs that indicate if the applicant is a U.S. Citizen will work - but most don't.
