Fake death certificate? Everyone involved at every step is in on it? Or, they just delay and say "oops our bad," after the 3rd?

I can 100% believe that he is not capable of returning and they are hiding behind HIPAA to slow walk it. But, that he's been dead this long and everyone is covering it up seems a stretch.
