Government gave Affidavit to Judge, who gave it to his Law Clerk.

Law Clerk had no idea what the document was and either didn't exercise "due diligence" or didn't check for inconsistencies, errors, etc. and then couldn't make recommendations to the Judge. The Judge trusted the wrong person for accuracy.
