Some simply choose not to fluoridate their tap water. Many of those add flouride to salt like we do with iodine. Some add flouride to milk.

Not all water utilities in the U.S add flouride either, the last two I have been on do not. Many people in my area of the state have their own water wells also. Here in Florida at least flouride is a naturally occurring element from the aquifer anyway, so it’s in most tap water whether they add more or not.
